SCREENTOCARE BY MAMACARE360
Terms of Use
Version 1.1. Effective October 5, 2026.
If you're in danger or thinking about harming yourself or your baby, call or text 988 any time, or call 911 if you're in immediate danger. For 24/7 support from trained counselors, call or text the National Maternal Mental Health Hotline at 1-833-TLC-MAMA (1-833-852-6262). ScreenToCare is not an emergency service. If you're not at your provider's office, your provider won't see your results until you send them and they check them.
These Terms of Use apply when you use ScreenToCare to complete a screening. By checking the consent boxes before you start, you accept them.
1. What ScreenToCare is
A tool that lets you complete validated mental health screening questionnaires on your phone, suggests next steps, and lets you send your results to your provider's office when you tap Submit. It's a screening, not a diagnosis, and it doesn't treat you or replace care from your provider.
2. Not for emergencies
No one monitors answers. If you're in danger, call or text 988 or call 911.
3. Your choices
Suggested next steps are options, not instructions. Decisions about your care are yours and your provider's. You can stop a screening at any time.
4. Who can use it
You must be 13 or older. If you're under 18, use ScreenToCare only when your provider's office gives you the screening.
5. Your phone
There's no account or login. Your name and answers stay on your phone, and ScreenToCare receives only anonymous counts. While you're in the middle of a screening, your answers are saved on your phone for up to 6 hours so you can pick up where you left off. When you tap Submit, your results are sent by regular email from your own email account. If you share your phone with someone, close the screen after you tap Submit. On the relationship safety questions, the Quick Exit button clears your answers and leaves the site.
6. If you screen through a doula
Doulas and other birth workers aren't medical providers, and results you send them may not be protected by HIPAA. You can also send your results to your OB, midwife or pediatrician.
7. Accuracy
Your results depend on your answers, and screening tools can miss things. If something feels wrong, talk to your provider, whatever your score.
8. Your information
Our Privacy Policy and HIPAA Notice explain how your information is handled.
9. Limits
SCREENTOCARE IS FREE TO YOU AND PROVIDED "AS IS." TO THE FULLEST EXTENT PERMITTED BY LAW, WE ARE NOT LIABLE FOR INDIRECT OR CONSEQUENTIAL DAMAGES, AND OUR TOTAL LIABILITY TO YOU IS LIMITED TO $100. NOTHING IN THESE TERMS LIMITS LIABILITY THAT CANNOT BE LIMITED BY LAW, INCLUDING FOR GROSS NEGLIGENCE OR WILLFUL MISCONDUCT.
10. Governing law
These Terms are governed by the laws of the State of Georgia, without regard to conflict-of-law rules, except where preempted by federal law. Any dispute will be brought exclusively in the state or federal courts located in Fulton County, Georgia.
11. Changes
We may update these Terms and will post changes here with a new effective date.
