SCREENTOCARE BY MAMACARE360

HIPAA Notice for Patients

Version 1.1. Effective October 5, 2026.

This notice explains how HIPAA, the federal law that protects health information, applies when you complete a screening through ScreenToCare.

Our role

ScreenToCare is designed so that it doesn't receive your health information. Your screening is scored on your phone, and your name and answers stay there. When you tap Submit, you send your results by email directly to your provider's office. Once there, your results are part of your medical record, and your provider is responsible for protecting them under HIPAA.

What ScreenToCare receives

Only anonymous signals that a questionnaire was started or completed through a clinic's link, which we add to monthly totals. They don't include your name, answers, scores or results. We don't sell information or use it for marketing.

If you send results to a doula

Doulas and other birth workers usually aren't covered by HIPAA, so results you send them may not have HIPAA protection. You can also send your results to your OB, midwife or pediatrician.

Your provider's privacy notice

Your provider has its own Notice of Privacy Practices, which explains all of your HIPAA rights. You can ask your provider for a copy.

Your rights

Under HIPAA, you generally have the right to see and get a copy of your health information, ask for corrections, get a list of certain disclosures, ask for limits on how it's shared, and ask to be contacted in a certain way. Because your results are held by your provider, send these requests to your provider's office.

Complaints

You can complain to your provider, to us at hello@screentocare.com, or to the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/complaints. You won't be penalized for filing a complaint.
